Why do GLP-1 drugs cost so much, and who pays for them?

In 2023 a month of Ozempic had a US list price of $936, against $83 in France.

GLP-1 drugs cost a lot, especially in the US, because patents delay cheaper generics and because sales are enormous. One source says the drugs have a median of 20 patents, and that patents and regulatory delay competitors and keep prices high. In 2023 the US of a month of Ozempic was $936, against $169 in Japan, $147 in Canada, $103 in Germany and the Netherlands, $93 in the UK and $87 in Australia. Semaglutide also had the highest earnings from US drug sales that year, with expenditures of US$38.6 billion.

Who pays is a moving target. In the UK, semaglutide is available on NHS prescription for diabetes at nominal or no cost to the individual, and for obesity for up to two years. In the US, coverage rules have shifted repeatedly: in April 2025 the Trump administration declined to finalize a Biden-era proposal for broad , and CHIP coverage of GLP-1s for weight loss, and in December 2025 it announced a Medicare bridge programme running from 1 July to 31 December 2026 at $50 per month for eligible beneficiaries. In February 2026 Novo said that from January 2027 the of Wegovy, Ozempic and Rybelsus would drop to $675 a month.

The prices raise real arguments. Cost is the highest barrier to GLP-1 use in the US, and 48.6% of people who stopped gave it as the reason. A 2026 review concluded the drugs are generally not compared with lifestyle interventions or no treatment from a payer's perspective, though outcomes vary drastically with its assumptions. Those are the review's findings, not a verdict on any person's choices.

Patents are running out unevenly: semaglutide's patent expired in India in March 2026, while in the US it is expected to become patent-free no earlier than December 2031.
