How does contact tracing stop an Ebola outbreak?

Ebola chains run through families, hospitals and funerals, so people who know each other can be found and watched for 21 days.

Which one is it?

Contact tracing stops Ebola by finding the people a sick person may have infected, watching them during the window when symptoms could appear, and isolating anyone who falls ill before they pass the virus on. It is the process of identifying people who may have been exposed to an infected person, then collecting more data to assess transmission, with the aim of reducing infections in the population.

For Ebola, it involves finding everyone who had close contact with infected individuals and monitoring them for signs of illness for 21 days, which matches the longest of 2 to 21 days. If any of these contacts comes down with the disease, they should be isolated, tested and treated. Then the process is repeated, tracing the contacts' contacts.

The contact-tracing loop
  1. Step 1: A confirmed case

A person is found to have Ebola

  2. Step 2: Find contacts

    Everyone who had close contact

  3. Step 3: Watch for 21 days

    Monitor for signs of illness

  4. Step 4: Isolate if ill

    Isolated, tested and treated

  5. Step 5: Repeat

    Trace the new case's contacts

The work is detective work among people who know each other. A study published in December 2014 found that transmission of the Ebola virus occurs principally within families, in hospitals and at funerals, and that the third person in any often knew both the first and second person. It concluded that the epidemic would not be as difficult to control as feared if rapid, vigorous contact tracing and were employed. On 17 December 2014, Sierra Leone launched "Operation Western Area Surge", and workers went door-to-door in the capital looking for possible cases.

Tracing links to the number R. If R is less than 1, an epidemic dies out; above 1 it keeps growing . Every contact found and isolated before they infect others pulls R down. The same method was used for smallpox, whose eradication was achieved by exhaustive contact tracing to find all infected persons.

It keeps growing

Challenge ยท Start with each case infecting 2 others, then lower R until each case infects fewer than one. Watch the rounds shrink instead of grow.

A simple model: the slider stands for R. Tracing and isolating contacts aims to lower it; the model does not show how many contacts are found.

  1. 1.Why are Ebola contacts monitored for 21 days?
  2. 2.What did the December 2014 contact-tracing study find about how Ebola spreads?
  3. 3.In the 2018 Kivu outbreak, what limited contact tracing?

Recap

Trace, watch for 21 days, isolate anyone who falls ill, then trace their contacts too.

๐Ÿ’ก A trick to remember it ยท Follow the footprints for 21 days: whoever stumbles gets cared for apart, and the trail ends there.

Surprising fact ยท Ebola contacts are monitored for 21 days, which matches the longest incubation period of 2 to 21 days.
